Overview

Evernorth Workplace Care is dedicated to improving the health and well-being of our patients. We are seeking a passionate and independent Registered Nurse to join our team as a Population Health RN. This role plays a key role in proactively identifying and engaging patients using data-driven tools and reports to improve health outcomes across defined populations.

The RN will work closely with the clinical teams and care coordination resources to ensure that patients who may be falling through the cracks are supported, re-engaged, and connected to appropriate care. You will play a pivotal role in delivering comprehensive care and empowering patients to achieve their healthcare goals.

Job Summary

Under the direction of a Licensed Physician and Clinical Director, the population health RN will provide general nursing care to patients for an employer's total population. The population health nurse will utilize their expertise and strong interpersonal skills to provide direct nursing care, and care coordination to a targeted patient population identified through data-driven reports. This role involves comprehensive patient health risk assessments, health education, and benefits navigation, requiring a self-directed and highly motivated individual who can thrive in an independent setting.

Major Duties: Include but are not limited to the following.

  • NURSING CARE:
    • Performs comprehensive health risk assessments and education
    • Follow established nursing protocols and procedures
    • Monitor medication compliance and provide patient education
    • Provide patient education on chronic conditions, focusing on abnormal biometric values and related behavioral lifestyle modifications
    • Utilize motivational interviewing techniques to promote sustainable behavior changes, including diet, exercise and stress management
    • Identify customer health education needs through targeted health assessment activities
    • Identify and facilitate overcoming psychosocial barriers to care
    • Collaborate with patients to establish health improvement plans, set personalized evidence-based health goals, and support patients in achieving those goals
    • Track and monitor screening compliance
    • Facilitate and promote preventive health screenings, including cancer screenings
    • Use frequent check-ins to reinforce understanding and adherence

  • CARE COORDINATION:
    • Facilitate communication and coordination with internal and external providers, including primary care, specialists
    • Coordinate with community providers and resources to address patients needs and care gaps
    • Offer concierge-style coordination of appointments, referrals, and follow-up care
    • Reinforce provider's care plans and ensure adherence
    • Advocate for patient needs within the care team and with matrix partners or systems
    • Go above and beyond to troubleshoot issues
    • Follow-up with patients regarding after-hours care recommendations

  • BENEFITS NAVIGATION:
    • Referral to clinical programs and services
    • Assist patients in navigating their benefits and accessing necessary resources
    • Advocate for patients to ensure they receive timely and appropriate care
    • Provide support for health-related site events, which include open enrollment, wellness committee facilitation, flu shot events, health fairs, etc
    • Serve as a trusted point of contact for patients and families-available for questions, reassurance, and step-by-step guidance
    • Participate in new hire orientation or wellness campaigns at onsite locations

  • ANALYTICS:
    • Work from reports and dashboards that identify patients, proactively engaging them in care management activities
      • Daily MD Live visit follow-up report
      • No PCP visit in 12+ months report
      • Missing biometric screening or preventative care markers report
    • Prioritized patient outreach and interventions based on risk stratification

  • ADMINISTRATIVE:
    • Charting within Electronic Health Record (Epic), in accordance with current policies and procedures

Qualifications:

  • RN with current unrestricted compact state licensure in Georgia required
  • At least three or more years of clinical practice experience required; 5 or more years of clinical practice experience preferred
  • Bachelor's degree in a health-related field preferred
  • Current BLS/CPR/AED Certification
  • Excellent Computer skills [specific software e.g. Word and Excel]
  • Excellent interpersonal skills. Must be able to work in a team environment and exhibit flexibility and enthusiasm in learning new information and developing new skills quickly
  • High energy level, with a passion for health improvement
  • Positive role model in demonstrating healthy behaviors
  • Demonstrate commitment to ongoing education preferred
  • Self-directness and the ability succeed in an independent role

This is a work from home role with periodic meetings and trainings onsite in our facilities in Northwest Georgia. This RN must reside in Northwest Georgia.

If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.

For this position, we anticipate offering an annual salary of 76,600 - 127,600 USD / yearly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
