What You'll Do:
- Evaluate and assess residents to develop individualized treatment plans
- Deliver hands-on therapeutic care focused on recovery, mobility, and independence
- Collaborate with a team of skilled nurses, CNAs, and therapy professionals
- Monitor and document progress while adjusting care plans as needed
- Educate residents, families, and staff on exercises and rehab best practices
- Provide care in a nurturing, long-term care environment focused on transition and healing
- Active California Physical Therapist license
- Experience in skilled nursing or long-term care preferred
- Strong interpersonal skills and a team-first mindset
