View Point Health is seeking a Supportive Employment and Education Specialist, who meets all DBHDD Policy and be trained in the IPS model, for an exciting program called the LIGHT Projects Early Treatment Program (ETP) and provides clinical services to clients throughout Gwinnett County. Early Treatment Program is a team based, multi-element approach to treating transition age youth and young adults, ages 16-30 years old with FEP (First Episode Psychosis). This position will be based out of our Dekalb/Fulton Office and will be a member of a team of professionals which include a Clinical Team Lead, Psychiatrist, Registered Nurse, Employment/Education Specialist, Case Manager, and Certified Peer Specialists.

CSC (Coordinated Specialty Care) is a collaborative, recovery-oriented approach involving consumers, treatment team members, family members (when appropriate), as active participants. CSC emphasizes shared decision making as a means for addressing the unique needs, preferences, & recovery goals of individuals served. CSC services are also highly coordinated w/ primary medical care, w/ a focus on optimizing consumers overall mental, emotional, & physical health.

Duties & Responsibilities:

Provide supportive employment and supportive education to all participants enrolled in the program.
Attend community educational & employment resources, trainings, & Continuing Education
Provide services to a caseload of at least 30 clients at any given time.
Understanding and adhering to the IPS (Individual Placement & Support) model.
Develop & Sustain community partnerships (including local high schools, colleges, educational institutes, & youth programs).
Develop & Sustain community partnerships (including local vocational services, employment resources, & local businesses).
Spends 50% or more of total scheduled work hours in the community, for example, meets with clients in community settings and develops relationships with managers at their businesses.
Provide job readiness skills related to resume building, hygiene, dress attire, interviewing, and etcetera.
Participate in weekly treatment team meetings to assist in the development of treatment plans with appropriate therapeutic interventions, notify professionals of any unusual observations or incidents, client progress, and follow up.
Report any program or client needs and concerns directly with team leader.
Follow insurer & agency guidelines when billing for services. Write precise, accurate, and clear progress notes in our electronic CareLogic system in a timely manner.
Maintain required records, documentation, & information in each client's file
Other related duties as assigned

Minimum Qualifications:
High school diploma/GED and three (3) years of job-related experience providing education, social or direct care services to persons with diagnosed special needs, or other populations; or two (2) years of experience at the lower level Training/Employment Tech 1 (SST050) or position equivalent.

Note: Must be at least 21 years of age and have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.

Preferred Qualifications:

Bachelor's degree of any Arts or Science
Training Specific to Individual Placement & Support model
At least 1 year of experience working with youth and young adults at transition age
At least 1 year of experience working with SPMI (Severe Persistent Mental Illness) population

Requirements/Competencies:

Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
Candidates for selection must pass a criminal background check (including fingerprinting)
Must pass a pre-employment drug screen and subject to random drug screens.
Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license.
