PRN Family Nurse Practitioner (Healthcare)

PRN Family Nurse Practitioner, Albuquerque, NM

About Us:
At Health by Design, we believe that compassionate care begins with seeing and honoring the whole person -- mind, body, and spirit.
As a faith-based, patient-centered primary care organization, we are more than a clinic -- we are a community committed to building meaningful relationships, improving lives, and guiding patients on their journey to long-term wellness. Our care model is rooted in empathy, prevention, and trust, with a focus on value-based care and lifestyle medicine that empowers individuals to lead healthier, more fulfilling lives.
We're changing what it means to provide healthcare by creating spaces that support healing, connection, and proactive care.

The Opportunity:
We are seeking a compassionate Family Nurse Practitioner (FNP) to join our team on a PRN ("as needed") basis at our outpatient family medicine clinic in Albuquerque. You will care for both pediatric and adult patients in a setting that promotes collaborative, whole-person care.
In addition to our brick-and-mortar clinic, you'll have the opportunity to serve in our Mobile Health Unit, providing care to City of Albuquerque departments and facilities -- expanding access and deepening community impact.

What Makes This Role Different:

Time to truly connect with patients: Appointments are 30-60 minutes so you can listen deeply, understand fully, and care intentionally.
No after-hours stress: Shifts are always between the hours of 8:00 AM-5:00 PM, Monday - Friday. No nights, no weekends, no call.
Supportive team-based care: Work alongside Registered Nurses and Care Coordinators in a collaborative environment that values empathy, presence, and purpose.
Focus on prevention: Engage in proactive care planning, patient education, and wellness support -- helping patients thrive, not just survive.

What You'll Do:

Provide holistic, age-appropriate care for acute, chronic, and preventive needs
Conduct thorough history and physicals with attention to the whole person
Order and interpret labs, diagnostics, and imaging
Prescribe both pharmacologic and non-pharmacologic treatments
Educate patients and families on lifestyle, prevention, and self-care
Collaborate with multidisciplinary care teams for seamless coordination
Support smooth care transitions and appropriate referrals
Serve patients with empathy, respect, and dignity at every encounter

What You'll Bring:

Master's degree from an accredited Nurse Practitioner program
Current, unrestricted New Mexico Nurse Practitioner license
National certification in your specialty
BLS and ACLS certification (PALS preferred)
Ideally, 1+ year of experience as an FNP in primary care (strong RN background considered for new graduates)
Excellent communication, listening, and teaching skills
Strong time management and teamwork capabilities
Above all: a heart for service, empathy, and a commitment to treating patients with compassion and respect
