Diabetes Educator (Finance)
100% FTE, Day Shift Role. Monday-Friday 8-4:30 (occasionally later 6 pm). You will work at the 1415 E. State St. Clinic in Rockford.

Qualifications
Bachelor of Science in Nursing or Bachelor of Science in Dietetics or related field from an accredited facility. Required
Work Experience
Functions as a clinical expert, resource and consultant for patients with diabetes.
Develops, plans, implements and evaluates education counseling for patients with diabetes and their families, staff and the community.
Provides appropriate referrals to community resources serving as a resource to the health care team.
Incorporates current Diabetes research findings into clinical practice and assesses, evaluates and recommends new therapy to enhance services while maintaining cost effectiveness.
